Australian Nursing Home Foundation is one of the Australia Government approved service providers, to provide community care packages that are individually tailored, planned and managed to help frail older people live independently in their own homes and enjoy quality of life for as long as possible.
There are three levels government subsidized home care packaged services depending on the assessed level of care to be provided:

Community Aged Care Packages (CACP) – equivalent to low care ( Hostel Level)

Extended Aged Care at Home Packages (EACH)– equivalent to high care ( Nursing Home Level)

Extended Aged Care at Home Dementia (EACHD) – equivalent to high care with dementia
In 2010, ANHF acquired funding from the Australian Government to provide home-based care services for a two-year pilot program, called Consumer Directed Care (CDC) under the Innovative Pool Program. The purpose of the program is to give care recipients and their carers to have greater control over the design and delivery of the care and services they receive.
Am I eligible to receive a packaged?
To be eligible for a care package, namely CACP, EACH, EACHD or CDC, the older people must first be assessed by an Aged Care Assessment Team (ACAT), to determine the level of care required and whether other special services is needed before they are referred to a community care service provider.
