Extended Aged Care At Home Packages


The Extended Aged Care at Home Packages (EACH) and Extended Aged Care at Home Dementia (EACHD) are funded by the Australian Government Department of Health and Ageing. These programs have been put together to support frail people, aged 70 or older, living in the community and eligible for high level residential aged care (nursing home level).
EACHD program specifically caters for people same as EACH and suffering from dementia.
These programs are flexible in contents and are coordinated by a registered nurse to deliver a full spectrum of high level care services ranging from nursing assessment and care planning; personal and domestic assistance; and access to behavioural management specialists so as to meet the high level complex care needs of the clients.
Services provided by EACH and EACHD include:

  • Personal assistance with bathing/showering, toileting, dressing

  • Domestic assistance with house cleaning, laundry services, preparation of meals and special diets

  • Assistance with shopping and transport to appointment / social activities

  • Respite care

  • Clinical care carried out by or under the supervision of qualified nurse or other professionals, such as complex wound management, medication management, urinary catheter care, stoma care, airways and tracheostomy care, care of naso-gastric tubes or enteral feeding

  • Referral to other appropriate health specialists and other service providers

  • EACHD also involves in the coordination and case management to support access to behavioural management specialists, psycho-geriatricians, geriatricians or mental health services to specifically address the behaviours of concern and psychological symptoms associated with dementia


Am I eligible to receive a packaged?

To be eligible to receive a EACH or EACHD package, older people must first be assessed by an Aged Care Assessment Team, to determine whether they are in need of high care. The assessment is free. Information on ACATs is available from doctors, hospitals and community centers, or to call Commonwealth Respite and Carelink Centre on 1800 052 222.
How is a EACH/EACHD package arranged?

Our Co-ordinator will assess your care needs and plan a package of service which will be tailored to meet your needs. We will discuss the care plan with you and your family or carer. After you have agreed on the care arrangement, we will ask you to sign the Agreement and Care Plan before service commence.
How much will I have to pay?

You are requested to pay the basic service fee. The maximum amount would not be more than 17.5% of the aged pension. You will have a monthly invoice for services you receive. No one will be denied services due to inability to pay.

 

EACH and EACHD offers continuum of care according to the client’s changing care needs and circumstances. ANHF employs qualified skilled and bilingual staff who will offer flexible hours care delivery and on call service. Comprehensive QA system will ensure service quality and consistency in care which meet the Australian Government standards.
