The Community Partners Program (CPP) was initiated in 2005 by the Australian Government Department of Health and Ageing. Its aims are to promote and facilitate increased and sustained access to aged care information and services by culturally and linguistically diverse communities with significant aged care needs Australian Nursing Home Foundation has been receiving funding to run this program for the Chinese community since 2006 and extended its services to the Indo-Chinese (Vietnamese) community since 2007. The target group of CPP is: Older Chinese / Indo-Chinese people who have the need to access aged care services. Aged care service providers who are keen to provide culturally appropriate care to Chinese / Indo-Chinese communities. We provide services include: Organise community information sessions / education programs to inform the Chinese / Vietnamese communities on aged care services, and how to access such services. Establish linkages among Chinese / Indo- Chinese community organisations, multicultural and mainstream aged care service providers to inculcate services to the targeted communities. Provide support and cultural specific briefings to Aged Care Assessment Teams and mainstream service providers to enhance culturally appropriate aged care assessment and ensure the most appropriate services are recommended. Identify current and future aged care needs of the Chinese / Vietnamese communities. Develop language clusters and support within mainstream aged care facilities; provide cultural specific briefings and advice on culturally appropriate care practices; set up supportive linkages among clustering facilities and ethnic communities. Work cooperatively with statewide Partners in Culturally Appropriate Care (PICAC) Program in promoting and advocating for culturally appropriate care services and developing resources on the delivery of culturally appropriate care.
