Call My Aged Care on 1800 200 422 or visit My Aged Care to apply for ‘Social Support Group’. You have to be assessed by the Regional Assessment Team for eligibility. If you need help, call us on 1800 882 288, and our friendly and professional staff will help you during the application process.

After you have been assessed and approved for ‘Social Support Group’ under the Commonwealth Home Support Program, you can contact the service provider. Our 11 Seniors Wellness Centres are located in various parts of metropolitan Sydney, activities are planned to support your wellbeing.
